(Q) Has Dr. Rosedale addressed anywhere the assertion (made by numerous keto or LCHF experts) that a significant percentage of LCHF dieters can't tolerate too much saturated fat, because their lipid/lipoprotein numbers become terrifyingly bad? Some have suggested that people who have had this experience should add carbs or take meds.

(A) The only number that really counts as far as LDL is the particle size. It is healthy to have large particles and unhealthy to have small, dense particles. I have never seen a patient on my diet whose particles did not enlarge. Sometimes, when the particles become larger (that is a good thing), the total weight may increase, and that is irrelevant.

The typical LDL and other cholesterol measurements are only total weight and should be ignored. There are specialty labs that will measure particle size and number, and that is all that should be ordered.
